Storage vMotion Phantom vm listed...no files on datastore

This is a new one for me.  Today I performed a storage vMotion on a server.  The server has multiple discs, but I only needed to move one of them.  The migration completed and I thought all was well, but then I noticed something weird.  That same server shows up as a vm on a totally separate datastore than I migrated from.  Not only that, but there are no files associated to the datastore.  Let me explain a little further...

esx_datastore_1 Original location of hard disc files

esx_datastore_2 New location files were successfully moved to

esx_datastore_3  Location which shows connected vm,  but has no associated files

I discovered the empty datastore after attempting to migrate everything back, but I received an error that the .vmdk file was missing.  I checked the settings if the vm...none of the discs reference the esx_datastore_3.  The server appears to be running with no problems, but I don't like the feeling that it appears to be connected to a datastore it has no files located on.  Any ideas?
